To speed things along when you arrive for your appointment and make it easier on yourself, PRINT OUT ALL PATIENT FORMS, complete them in ink and have them ready to hand to us when you arrive at Columbia Eye Clinic for your scheduled appointment.

Please note our new No-Show and Late Cancellation Policy: To help us serve all patients efficiently, beginning October 5, 2026, we will implement a no-show and late cancellation fee for failure to keep an appointment without prior notification or cancelling/rescheduling an appointment with less than 24 hours notice.
